האם פחמימות הן אכן הבעיה?

בעידן שבו כל המידע התזונתי הוא במרחק הקלקה אחת, לא נדיר לשמוע טענות כמו: "פחמימות הן הבעיה", "אינסולין הוא שאחראי להשמנה", או "הקלוריות אינן חשובות אם אתה עושה דיאטה קטוגנית". אז לפני שנבהיר מדוע כל הטענות הללו שגויות, עלי להדגיש כי אין לנו דבר נגד דיאטות דלות פחמימות ושאפילו אני משתמש בהן ככלי עבור הספורטאים שלי כשאני מנסה לשפר את הרכב הגוף שלהם. מכל מקום, הסיבה איננה משום "היתרון המטבולי" לעומת דיאטות אחרות, אלא מסיבות הרבה יותר פשוטות.

יצירת גרעון קלורי

רבים שטוענים נגד תיאוריית הקלוריות נכנסות- קלוריות יוצאות נופלים למלכודת האמונה כי אנחנו עוסקים במערכת קבועה, בעוד שלמעשה אנחנו מדברים על מערכת דינמית שהגוף שלנו משנה את תגובותיו ביחס אליה בהתאם לכמות הקלוריות הנצרכת. במילים אחרות, כמות הקלוריות שאנחנו מעכלים משפיעה על כמות הקלוריות שאנחנו מוציאים. במספר מחקרים (6-1) הוכח כי כשאנו מעלים את כמות הקלוריות שאנו צורכים (ובכך מעלים את משקל הגוף) הדבר עשוי להוביל לעלייה בסך האנרגיה שאנו מוציאים ביום (TDEE), בעוד שכשאנחנו מורידים במספר הקלוריות שאנו צורכים אנחנו מפחיתים את כמות הקלוריות שאנו שורפים ביום. בדרך זו הפחתת המשקל הממשי לטווח הארוך תהיה נמוכה מהמצופה (7) בלי קשר לשאלה האם אנחנו בדיאטה דלת או מרובת פחמימות. גם כך, עלינו לקחת בחשבון שהתנהגות האדם תשפיע יותר על תוצאות הפחתת המשקל שלו מאשר האדפטציה המטבוליות.

בהתייחס להתנהגותם של אלה שעושים דיאטה, נמצא כי רובם מעריכים הערכת-חסר את כמות הקלוריות שהם צורכים ומעריכים הערכת-יתר את כמות הקלוריות שהם שורפים (13-8), למרות שהם יודעים כי החוקרים עצמם צופים בהם (14). מצד שני, אנו מוצאים כי כשהאדם מודע לכמות הקלוריות שהוא מוציא, הוא מרגיש חופשי יותר לצרוך מזונות טעימים (15). בהתחשב בעובדה שכיום רוב בני האדם משתמשים באמצעים לבישים שמעריכים הערכת-יתר את הוצאת הקלוריות שלהם במהלך אימון (16), ההבדל שבין הקלוריות המדווחות למול הצריכה הממשית של קלוריות גדול אפילו עוד יותר. זו אחת הסיבות לכך שאנו מוצאים חוסר אחידות כשאנו משווים דיאטות דלות פחמימות בהשוואה לדיאטות דלות שומן במצבים שבהם הנבדקים חופשיים לעשות מה שהם רוצים.

מגבלה נוספת היא שבמחקרים רבים שנהגו להעדיף דיאטות דלות פחמימות הייתה יותר צריכת חלבון, ולפיכך אנחנו לא מדברים על דיאטות עשירות פחמימות מול דיאטות עתירות שומן, אלא על דיאטות עתירות חלבון לעומת דיאטות דלות חלבון. זהו גורם עיקרי, משום שכשאנחנו מעלים את כמות החלבון, אנחנו מאבדים פחות מסת שריר, התיאבון שלנו יורד ואנחנו אפילו מעלים את ה-TDEE (23-17).

אם כך, מה קורה כשאנחנו מגבילים את כמות הקלוריות והחלבון בשתי הדיאטות?

כשאנו בוחנים את המחקרים שבהם שני הפרמטרים הללו נשלטים, נמצא כי אין הבדלים משמעותיים ביניהם. Golay ועמיתיו (24, 25) דיווחו כי כאשר צריכת החלבון והקלורית היו זהים במהלך מצב של גרעון קלורי גדול, לא היה כל הבדל בהפחתת השומן בין אם צרכנו 15% או 45% מסך כל האנרגיה הנצרכת בצורת פחמימות. Hall ועמיתיו (26) הגיעו לאותה מסקנה במחקרם, בו דיווחו על היעדר הבדל בהפחתת השומן בין דיאטה קטוגנית לדיאטת עם כמות פחמימות מתונה כשכמות הקלוריות והחלבונים היו תואמים. יתר על כן, במחקר מטא אנליזה שבוצע ע"י Johnston ועמיתיו (27) בדק דיאטה קטוגנית מול דיאטה עם כמות פחמימה מתונה עם אותה כמות חלבון וקלוריות, ומצא כי ההבדל בין שתי הדיאטות היה מזערי – 0.2 ק"ג במשך שנה שלמה. המשתתפים בקבוצת הדיאטה הקטוגנית איבדו יותר ממשקלם (מגליקוגן ומים) אבל איבדו את אותה כמות שומן.

וכאן נמצאת הבעיה האמיתית: כל דיאטה נכשלת בטווח הארוך אם לא מתמידים בה

לא משנה מה כמות הפחמימות הנצרכת בדיאטה, אנו רואים כי בתקופה שבין חצי שנה עד שנה רוב האנשים שבים ועולים במשקל שהפחיתו (אדפטציה מטבוליות, היעדר התמדה בדיאטה, יותר רעב…). וכך, נטייתם להאשים את הפחמימות כגורם היחיד להשמנה כלל אינה הגיונית.

מצד שני, האם יש הוכחה לכך שתוכלו לאבד משקל רב בעודכם צורכים כמות רבה של פחמימות? התשובה היא כן!

לצורך כך עלינו להיזכר בשנות ה-40 של המאה הקודמת כשהדיאטה של וולטר קמפנר, שנודעה בשם "דיאטת האורז", עוררה תשומת לב רבה. הדיאטה הזו מבוססת באופן בלעדי על אורז, ירקות ופירות, המביאים לצריכה של מעל 90% פחמימות. גם אם נתעלם מהעובדה שאנחנו לא מדברים על דיאטה "אופטימלית" משום שהיא דלה מאוד בחלבון ושומן, היא באמת מסייעת לנו להבין כיצד צריכת פחמימות מעל 500 גרם עשויה לגרום לאיבוד של יותר מ-45 ק"ג משקל הגוף (בחלק מהמקרים של אנשים ששקלו יותר מ-100 ק"ג) אם הם נמצאים בגירעון קלורי (28).

לאחרונה למדנו על אוכלוסיות – כמו בני האי קיטווה בפפואה גיניאה החדשה, או תושבי אוקינאווה – שצריכת הפחמימות שלהם היא מעל 70 ו-90 אחוז (בהתאמה) מסך הצריכה הקלורית שלהם, ואיכשהו הם לא סובלים מהשמנת יתר, וגם לא ממחלות אופייניות למאה ה-21, משום שהצריכה הקלורית שלהם איננה גבוהה (35-29).

בסופו של דבר, המאזן האנרגטי הוא שקובע האם איבוד השומן יהיה גבוה או נמוך, ולא השאלה האם אתם מקיימים דיאטה עם כמות פחמימות גבוהה או נמוכה. התמדה בדיאטה היא אבן פינה חשובה אם ברצוננו להשיג ירידה ברקמת השומן בטווח הארוך, וכדי להשיג זאת עלינו ללמד אנשים כי בין אם הם מקיימים דיאטה עתירת פחמימות או עתירת שומן, הם יוכלו להשיג את מטרותיהם כל זמן שיקפידו על גירעון קלורי וימצאו דיאטה שתתאים להם בטווח הארוך.
